Dual eligible individuals are generally vulnerable to complex medical and social needs. Organizations who care for these individuals are often challenged by their complex array of needs, coordinating care across providers and community services, and difficulties in integrating Medicare and Medicaid programs. To ensure better care and outcomes for dual eligibles, the Center for Medicare and Medicaid Services (CMS) has developed programs focused on greater integration and alignment between programs, improved quality and enhanced beneficiary experience.
